Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

1. Our pledge

LiveGBX is committed to protecting the privacy of Protected Health Information (PHI) that we receive from your healthcare provider or that you provide to us. We are required by law to maintain the privacy and security of your PHI and to give you this notice.

2. How we may use and disclose PHI

• Treatment: to coordinate or manage your healthcare.
• Payment: to obtain payment for services.
• Healthcare operations: quality assessment, training, accreditation, audits.
• As required by law: public health, court orders, law enforcement.
• With your authorization: any other use requires your written authorization, which you may revoke at any time.

3. Your rights

• Right to inspect and copy your PHI (§164.524).
• Right to request amendment (§164.526).
• Right to an accounting of disclosures (§164.528).
• Right to request restrictions on uses and disclosures.
• Right to request confidential communications.
• Right to a paper copy of this notice.
• Right to file a complaint without retaliation.

4. Our duties

We are required to maintain the privacy of PHI, provide this notice of our duties and privacy practices, abide by the terms of the notice currently in effect, and notify you in the event of a breach of unsecured PHI (45 CFR §§164.400–414).

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at privacy@livegbx.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

6. Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for all PHI we maintain. The current notice is always posted at this URL with its effective date.
